Sick (fit) notes

If you’ve been off sick for more than 7 days

To request a fit note, phone us on 01747 840226.

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm it yourself by filling in a form when you return to work. This is called self-certification.

Find out more about fit notes on the NHS website.

If you go to hospital

If you go to hospital, or have a procedure or operation there, the hospital doctor can give you a fit note (also called a Med 3).

Fit notes are needed for Statutory Sick Pay and benefits if you cannot work, or if you may be fit for work with support from your employer. The doctor who is responsible for your care at the time must provide it. So if a hospital doctor tells you not to return to work, it is their responsibility to give you a fit note.

We can often email your fit note to you if we have a verified email address on your record.
