Register with the surgery

Catchment area

Before you register, please check that you live in our catchment area.

You can also find out which GP surgeries you can register with on the NHS website: Find a GP (nhs.uk).

How to register

To register with the surgery:

  • if you are 13 or over, fill in a new patient registration form (13 and over)
  • if you are 12 or under, fill in a new patient registration form (child under 13)

It helps if:

  • you can provide proof of identity, ideally photo ID
  • for anyone under 18, reception can take a copy of their vaccination record from their red book

Your contact details

The mobile number on your record should be your own. For patients under 16, we can record a parent’s mobile number and email address, but we will remove these when the patient turns 16.

When you turn 16, please contact us to update your record with your own mobile number and email address.

After you register

The practice nurse reviews all the medical records we receive and may contact you to offer an appointment if it would benefit you.

We encourage all new adult patients to book a new patient check with the practice nurse. This appointment takes about 20 minutes and gives us a baseline for the future.

If you take regular repeat medicine or have ongoing health problems, please book a telephone appointment with a GP as soon as possible, so there is no delay in getting the medicine you need.

If you have any questions, please ask reception. We hope you will not need us often, but we will always do our best to help if you do.
