Your health record includes information such as any conditions or allergies you have and any medicines you take.
Most patients now automatically see more information in their online record, including letters, test results and appointment notes. You can also nominate someone you trust to access your record.
You can see your health record in your NHS account (through the NHS website or NHS App) or in SystmOnline. If you can only see your medicines and allergies, ask the surgery for online access to your full record.
Requesting access to your full record online
First, you need to register for online services and prove who you are:
- bring photo ID to the surgery so we can confirm your identity. If you showed photo ID when you registered, you can phone us instead to set up online services
- if you are setting up access for the first time, we will give you a username and password
- log in to SystmOnline or the app
